Canada Medical Inadmissibility 2025: Types, Cost Threshold, and Exemptions
When applying for a Canadian study permit, work permit, or permanent residency (PR), every applicant must meet the medical admissibility requirements set by Immigration, Refugees and Citizenship Canada (IRCC). If an applicant is deemed medically inadmissible, they may be refused entry or denied immigration status in Canada.
Understanding how medical inadmissibility in Canada works is essential for avoiding unnecessary refusals and preparing a strong immigration application.
What is Medical Inadmissibility in Canada?
Medical inadmissibility occurs when a foreign national’s health condition may:
1. Pose a Danger to Public Health
Applicants may be considered inadmissible if they have an illness that could put the Canadian population at risk.
Examples include:
- Active infectious diseases like tuberculosis or untreated syphilis
- Conditions that require public health intervention or quarantine
2. Pose a Danger to Public Safety
If a person’s medical condition could lead to violent, unpredictable, or dangerous behavior, they may be denied entry.
Examples include:
- Severe uncontrolled psychiatric disorders
- Conditions causing sudden incapacitation, such as severe epilepsy
3. Cause Excessive Demand on Health or Social Services
Applicants may be refused if their medical condition is expected to cause excessive strain on Canada’s publicly funded healthcare or social services.
Examples of conditions that may trigger excessive demand:
- Chronic illnesses requiring costly ongoing treatment (e.g., kidney failure requiring dialysis)
- Developmental disabilities needing specialized long-term care (e.g., autism, Down syndrome)
- Dependence on expensive prescription medications (e.g., hemophilia treatments)
Excessive Demand Cost Threshold 2025
Each year, IRCC sets an excessive demand cost threshold to determine whether an applicant’s healthcare or social service needs are higher than the Canadian average.
📌 As of January 1, 2025, the cost threshold is CAN$27,162 per year.
Previous thresholds:
- 2024: CAN$26,220
- 2023: CAN$25,689
- 2022 (Mar 16–Dec 31): CAN$24,057
- Before Mar 16, 2022: CAN$8,019
💡 What this means: If your projected healthcare costs exceed CAN$27,162 annually, IRCC may find you medically inadmissible under the Immigration and Refugee Protection Regulations (IRPR).
Who is Exempt from Medical Inadmissibility Due to Excessive Demand?
Certain groups of applicants are exempt from the excessive demand rule, including:
✔ Refugees and Protected Persons
✔ Sponsored spouses, common-law partners, and dependent children
⚠️ However, these exemptions do not apply to cases involving public health or public safety risks.
How IRCC Assesses Medical Admissibility
- All PR applicants (and some work/study permit applicants) must undergo a medical exam with an IRCC-approved panel physician.
- If issues are found, IRCC may issue a Procedural Fairness Letter (PFL), giving applicants the chance to provide additional medical evidence or a mitigation plan.
How to Overcome Medical Inadmissibility in Canada
If you’re found medically inadmissible, you still have options:
✅ Submit a Mitigation Plan – Show how you can cover the costs of treatment privately (e.g., through personal funds or private insurance).
✅ Request a Procedural Review – Provide updated medical reports or expert opinions to challenge the decision.
✅ Apply for an Exemption – In some cases, humanitarian and compassionate (H&C) grounds may allow exceptions.
